Most of our care happens in the community, in people’s homes, over the phone and through outpatient appointments. Our multidisciplinary team provides specialist palliative and end of life care in the place that suits people best.
We care for people from across Hastings and Rother. From Camber in the east to Ticehurst in the north, Little Common and Cooden in the west, and everywhere in between.
Our Community Outreach team provides specialist palliative and end of life care, support and advice across Hastings and Rother.
The majority of the care we provide happens in people’s homes, over the phone and through outpatient appointments, helping people receive the care and support they need in the place that suits them best.
Patients benefit from the expertise of a wide range of health and care professionals working together to provide specialist care and support in people’s homes and communities, not just at the Hospice.
Our team can help with everything from symptom management and advance care planning to emotional support, coordinating care and accessing other Hospice services when needed.
Alongside supporting people living with life-limiting illness, we also provide advice, guidance and support for families, friends, carers and other healthcare professionals.
No. Hospice care works alongside the care provided by a GP, hospital team and other health and care professionals. These teams work together to make sure care is coordinated and tailored to the patient’s needs.
There isn’t a set number of visits. The support we provide depends on individual needs and circumstances and might be provided at home, through outpatient appointments at the Hospice or over the phone. Some people need regular contact for a short time, while others may only need support at key points during their illness.
